Ja'Onni McClerkin

Ja’Onni McClerkin serves as the Social Care Navigator for the Buffalo Center for Health Equity. Before joining the team, Ja’Onni gained experience in administration, scientific research, public policy, and customer service. She also brings expertise in governance and strategic oversight within complex organizations. As Social Care Navigator, Ja’Onni conducts Health-Related Social Needs (HRSN) screenings for Medicaid members, completes eligibility assessments for individuals with identified social needs, and connect eligible individuals with community-based service providers through referrals.

Outside of work, Ja’Onni enjoys dancing, visiting art museums, and exploring new cafés around the city. A Buffalo native, her favorite sports team will always be the Buffalo Bills! A perfect day for Ja’Onni includes enjoying an iced matcha latte and sweet treat with any novel by Toni Morrison or Octavia Butler.
